The Population Health Management (PHM) models continue to become a preferred vehicle for improving quality and outcomes and reducing costs. Long term/post-acute care (LTC/PAC) providers like yourself are taking ownership and looking to lead the way.

This summit comes at a critical time and will bring together LTC/PAC providers, health plans, regulators, and investors for a two-day intensive to explore the ways LTC/PAC providers are driving change and integrating new models of care within their organizations using PHM strategies.
